Does QMB Cover Ambulance and Emergency Medical Transportation?
By Sharon Ben-Moshe · Founder & Editor
Qualified Medicare Beneficiary coverage pays the Medicare Part B deductible and 20% coinsurance for medically necessary emergency ground and air ambulance transports. Under CMS Medicare ambulance billing regulations, Medicare-participating ambulance suppliers cannot bill a QMB enrollee for any cost-sharing balance, protecting patients from unexpected emergency transportation bills.
Ambulance coverage rules under QMB
- QMB pays the 20% Part B coinsurance for medically necessary emergency ambulance trips.
- Federal balance-billing protections legally shield QMB patients from emergency ambulance balances.
- Air ambulance transports that meet Medicare medical necessity standards are covered with zero copay.
- QMB Only does not cover routine, non-emergency transportation to doctor visits.
When Medicare Part B covers ambulance rides
Medicare Part B covers emergency ambulance transportation when travelling by standard vehicle would endanger your health and you require transportation to the nearest appropriate medical facility equipped to treat your condition. Medicare covers both emergency ground transport and emergency air ambulance (helicopter or airplane) when rapid transport is medically essential.
After the annual Medicare Part B deductible ($283 in 2026), Medicare pays 80% of the Medicare-approved fee schedule amount. The remaining 20% coinsurance is the responsibility that QMB covers.
Balance-billing protections for ambulance services
Emergency medical situations frequently involve municipal fire departments, private ambulance operators, or hospital-based response teams. Under federal statute (42 U.S.C. 1396a(n)(3)), any ambulance service that participates in Medicare must accept the Medicare assignment and is prohibited from billing a QMB patient:
- Assignment requirement: The ambulance company bills Medicare first and receives payment under the Medicare ambulance fee schedule.
- State crossover: The remaining 20% coinsurance crosses over to state Medicaid for processing.
- Prohibition on patient billing: Even if the state Medicaid payment is less than the full 20% coinsurance amount, the ambulance provider cannot bill the patient for the difference.
- Disputed bills: If a private collection agency sends a bill, providing proof of QMB status mandates the immediate cancellation of the balance.
Non-emergency medical transportation distinctions
QMB Only covers Medicare cost-sharing for Medicare-covered services. Because Medicare generally excludes non-emergency medical transportation (such as scheduled rides to dialysis or specialist consultations, unless certified by a physician as bed-confined), QMB Only does not pay for non-emergency vans or taxis. Beneficiaries with QMB Plus or full Medicaid may access routine medical rides through their state Medicaid transportation broker.
