Tennessee Medicare Savings Program (MSP) 2026: TennCare Limits and Rules
By Sharon Ben-Moshe · Founder & Editor
Tennessee assists low-income seniors and individuals with disabilities through Medicare Savings Programs administered by TennCare, the state Medicaid agency. These programs help older Tennesseans on fixed incomes retain more of their monthly Social Security benefits by transferring premium and medical cost obligations to the state.
TennCare MSP income limits for 2026
Tennessee follows federal poverty guideline percentages for its three primary Medicare Savings Program tiers:
- Qualified Medicare Beneficiary (QMB): Monthly income ceiling of $1,350 for an individual or $1,824 for a couple. TennCare covers Part A and B premiums, annual deductibles, and doctor coinsurance charges.
- Specified Low-Income Medicare Beneficiary (SLMB): Monthly income ceiling of $1,616 for an individual or $2,184 for a couple. TennCare pays the standard Medicare Part B monthly premium.
- Qualifying Individual (QI): Monthly income ceiling of $1,816 for an individual or $2,455 for a couple. Covers the full Part B premium, renewed annually subject to federal allocation.
Tennessee asset limits and exempt property
Tennessee enforces the standard federal resource test. Countable assets cannot exceed $9,950 for an individual or $14,910 for a couple. Countable resources include checking and savings balances, certificates of deposit, non-residential real estate, and brokerage holdings. The state excludes your home, one vehicle, household furnishings, and small life insurance policies with face value under $1,500.
Coordination with TennCare managed care organizations
When you qualify for QMB in Tennessee, your coverage coordinates through the TennCare system. Participating medical providers bill Medicare first and bill TennCare for the remaining coinsurance. Providers cannot seek unpaid balances from QMB patients under federal billing restrictions.
